Eligibility Criteria:
To qualify for Housing Stabilization Services, individuals must meet the following criteria:
- Medical Assistance Recipient: Must be eligible for and enrolled in Medical Assistance (MA).
- Disability or Age: Must have a documented disability or be aged 65 or older.

Housing Instability: Must meet one of the following conditions:
- Experiencing homelessness or at risk of homelessness.
- Living in an unstable housing situation.
- Moving from an institution or foster care setting to a more independent living arrangement.
- Need for Support Services: Must require assistance with transitioning to and maintaining stable housing.

Enrollment Process:
Follow these steps to enroll in Housing Stabilization Services:

Eligibility Assessment
Contact HELPINGHANDS Solutions LLC to conduct an initial assessment to determine eligibility.

Submit Eligibility Request Form
We will submit a Housing Stabilization Services Eligibility Request Form (DHS-7948) to the Minnesota Department of Human Services (DHS).

Review Process
DHS will review the submitted form and supporting documentation. The review will be conducted in the order in which the forms are received.

Notification of Determination
DHS will notify us of the eligibility determination through the MN-ITS mailbox and will also send an email to the client informing them of the outcome. We will also keep you updated on the outcome.

Start of Services
If approved, you will begin receiving Housing Stabilization Services based on your individualized plan.
